Preventing exacerbations is a key goal in the management of patients with COPD because these events are associated with increased morbidity, mortality, and more rapid decline in pulmonary function. While bronchodilator and bronchodilator plus inhaled corticosteroid treatment significantly decrease exacerbations, they are not effective in all patients and new COPD therapies are urgently needed. The first Hot Topic paper describes the efficacy of roflumilast, a new COPD treatment, in decreasing exacerbations and the successful use of a strategy for identification of a patient phenotype with increased likelihood of responding to this therapy.
